Nurse to patient ratio is an important topic for healthcare professionals, administrators, and patients alike. Improving patient outcomes is one part of the quadruple aim that healthcare providers and hospitals continue to strive to achieve (Melnyk et al., 2018). The clinical issue of interest I have selected is: The impact of nurse-patient ratios on patient outcomes. My PICO(T) question is: How are patients in an inpatient acute-care hospital setting (population) impacted by instituting minimal nurse-patient ratios based on patient acuity (intervention) in comparison to hospitals without minimal nurse-patient ratios (comparison) when considering improving standard patient outcomes (outcome) over the period of one year (time)?
Two of the databases I used were MEDLINE and PubMed. When I first entered my topic into the library database MEDLINE, my search only managed to yield 4 results. I realized I needed to change my search. When I entered the key words nurse-patient ratios I received 6,687 articles. Of those articles, led than half could be considered original research and even less met the criteria of being current. According to one article, correctly using tools such as Boolean operations when conducting research can assist healthcare professionals find the right evidence to answer clinical questions (Bramer et al., 2018). The next step was for me to enter the Boolean operation and; I used this particular operation with the phrase patient outcomes. This combination gave me 576 results and again less than half of those results could be used as original research. I continued to use various Boolean operations and change the search criteria to refine the results. I quickly realized I would need to practice navigating the information to generate the best results. One important strategy is setting up the search. To produce the best results, it is imperative to set up the search. For example, making sure to conduct the search under the correct subject, in this case use the subject of nursing will focus the results to be more relevant. Clinicians are increasingly encouraged to questions current clinical practices and use evidence-based information to change the status quo (Bourgault et al., 2018).

The opioid use in the United States is one of the most challenging aspects of the healthcare system. Working in the healthcare domain brings attention to opioid use in the United States as it became one of the most clinical issues faced in most practices. There are currently many State regulations on prescribing opioids. Opioid use disorder is very common among adult patients who experience chronic pain due to musculoskeletal problems. Most patients with opioid use disorder end up in Suboxone clinics to be treated for their use disorder. However, this treatment plan is not an option for all patients due to insurance coverage and patients’ acceptance of their use disorder.
PICOT question includes patient population, interventions, comparison of groups outcomes and timeframe (Melnyk, 2018). PICOT format will help in formulating my clinical question about the effect of opioid use disorder on adult patients who 50 years are and older. In which – my population will be patients with opioid use disorder who are 50 years and older and the intervention or the issue of interest will be opioid use disorder for chronic pain compared to acute pain in patients with musculoskeletal problems. The outcome will be the results of studies done in recent years however the timeframe will be the lengths of the studies. This PICOT format will help make sure the study is covered from all sides and the question is completely answered.
I used the University library to find peer reviewed articles which is related to the opioid use in the United States. The database includes a lot of systemic review articles which are filtered by databases like CINAHL plus with full text. Also, the midline with full text databases has advanced limited option for systemic review articles (Walden, n.d). CINAH L database combined with midline database full text is the main search database I used for the purpose of my clinical issue which is opioid use returned 30,104 articles which was narrowed down with the addition of the Boolean search parameter such as adding a specific area such as the United States and as well as limiting population to adults who are 50 years and older. It is very important to use several search words when looking for a specific topic to get the best results. Another strategy that will help with my study is to use most of the year of publication when choosing an article.
