Abstract
The purpose of this project is explaining the benefit of early patient education regarding preeclampsia in African American women at the OB/GYN Center to reduce the number of hospital readmissions by providing education on the signs, and symptoms, potential complications, recommended lifestyle changes, and a timeframe for when they should seek medical attention. Due to the racial disparities in maternal health and the rise of maternal mortality rates in African American women, the project will be implemented at the OB/GYN Center in conjunction with the postpartum/women’s specialty floor at the hospital. The most important finding of my capstone project is that implementing a purposeful early education initiative reduces hospital readmission rates due to preeclampsia which leads to decreased incidences of maternal mortality and improved patient outcomes and quality of care.
Introduction
My capstone project addresses the need to reduce the number of hospital readmissions due to preeclampsia by implementing an early patient education program that will cover the signs, and symptoms of preeclampsia, potential complications, recommended lifestyle changes, and a timeframe for when they should seek medical attention. The target population for my project is prenatal and postpartum African American women at the OB/GYN Center. The major components of my intervention plan include an organizational committee dedicated to the management and treatment of preeclampsia, a community liaison, a home-visit nurse, and patients and family members willing to participate. Pursuing my intervention plan is important because it ensures all staff will be involved in improving the health status of the patient and the quality of care they receive at the OB/GYN Center. The major components of my implementation plan include interprofessional collaboration with internal and external stakeholders, and communication, and delegation from the leadership/management team to the entire staff. Delegating responsibilities to the staff will establish prompt treatment towards the timeframe and goals of the project. The major component of my evaluation plan is measuring the reduced number of hospital readmissions secondary to the comparison of the patient’s pre-assessment and post-assessment knowledge of preeclampsia and their completion of the early education program prenatally throughout their postpartum period.
