Assignment Description

In this assignment, you will reflect on professional practice, theory, intervention and evaluation skills, values, and knowledge as they align to the Council on Social Work Education (CSWE) core competencies. Using the previously submitted assignments and feedback from the instructor, you will compose a professional practice, theory, intervention, and evaluation portfolio.

Assignment Instructions

To ensure one document for the portfolio, develop one Word document that outlines and includes the following analyses related to the client problems in your agency.

 

1. Title Page.
2. Table of Contents.
5. Paper 1. Integrate references into the final references page(s).
6. Paper 2. Integrate references into the final references page(s).
7. Paper 3. Integrate references into the final references page(s).
8. Termination and Follow-Up.
   • Describe the termination process and plan you have developed with your client, including relevant ethical standards that informed the process, and indicate whether the client will end the services you provided with the agency or whether services will be transferred to another professional as part of the transition process.
   • Describe your client’s progress with respect to the intervention goal you established and how you measured this progress.
   • Describe your follow-up plan with the client, including the support services that are needed to ensure success of your intervention goals and evaluation measures.
   • Describe the inter-professional collaboration efforts you included as part of work with your client, including advocacy efforts.

• Evaluation and Reflection.
  • Discuss at least one social policy change you believe is needed to best promote client well-being, rights, and social, economic, and environmental justice at your practicum site.
  • Describe your experience with administrators, staff, interdisciplinary professionals, and the community.
  • Analyze how the supervision provided to support your work as an intern influenced your practice and learning opportunities.
  • Analyze how the Professional Portfolio project and the practicum experience helped you identify your professional strengths and challenges. Be certain to indicate the strengths and challenges as part of your narrative.
  • Explain how your new knowledge from this assignment and the practicum experience will inform your future practice.
  • Discuss what you hope to gain in your Advanced Practicum experience.
